Current Developments in the Prevention and Improvement of Intestinal Disorders: A Mini-Review

Particularly, Irritable Bowel Syndrome (IBS) patients and Inflammatory Bowel Disease (IBD) patients, which include Ulcerative Colitis (UC) and Crohn’s Disease (CD), are on the rise, especially in the young generation. Behcet’s disease (an autoimmune disease) and bowel obstruction are also common intestinal disorders. Furthermore, colorectal cancer, including colon and rectum cancer and small intestinal cancer, are the typical disorders in the intestine. Other disorders in the digestive tract are infectious diseases like Helicobacter pylori infection. Even though symptomatic treatments have been increasing for the treatment of intestinal disorders, the ways of improving and preventing these diseases are still controversial.
The progress of medicine and treatment is rapid. However, recent approaches to the prevention and improvement of these intestinal disorders are suppressing dysbiosis and preventing chronic inflammation. This mini-review discusses the hypothesis of whether the improvement of the diet is a preferable choice for the prevention of these intestinal disorders. Dietary interventions are beneficial for the prevention and improvement of intestinal disorders since the first approach to intestinal disorders is dietary intervention. The Mediterranean diet, the diet from the 5-a-day campaign, and the Japanese diet are well-known healthy dietary strategies. A healthy diet regimen is not only beneficial for the prevention of intestinal disorders but also a useful strategy to reduce stress and ameliorate mental illness. In addition, the intake of phytochemicals is good for keeping healthy gut microbiota and preventing intestinal disorders. Furthermore, vitamin D3 intake with these phytochemicals works as an adjuvant to improve gut microbiota and upregulate immune responses. As a result, the decreasing production of TNF-α ameliorates chronic inflammation and intestinal disorders at an early stage.
In recent years, prevention of the non-disease condition “ME-BYO” has been a popular approach for healthy and long living in Japan. This idea prevents the manifestation of diseases before the onset and is also applicable to intestinal disorders. This mini-review discusses ways of preventing and ameliorating intestinal disorders.

Nursing Coaching Can Improve the Quality of Life and Immune-Endocrine Condition in Hospitalized Cancer Patients
IntroductionAny cancer diagnosis induces fear and shocking emotional experiences accompanied by anxiety, depression, unpredictability, and distress. The emotional effect of a cancer diagnosis and the rigidity of cancer treatment negatively impact the quality of life (QoL) of patients, and this may continue after treatment. Additionally, emotional distress induces neuroendocrine stress activation systems and raises stress hormone secretion by causing immunological dysfunctions. The present narrative review aims to describe nursing coaching approaches that improve QoL perceptions among cancer patients during their hospitalization.
MethodsThis review was carried out using the PRISMA methodology until the end of November 2023 through PubMed, Scopus, Web of Science, and CINAHL databases. Researchers systematically collected all the currently available literature. The search terms and boolean operators used to combine keywords were: “QoL” AND “hospitalization” AND “cancer patients” AND “nursing coaching”.
ResultsFour manuscripts were selected in the present review. One manuscript belonged to the British Nursing Database and was a mixed-block-randomized study; one belonged to Scopus, which was also in the PubMed, WoS, and Medline and was a study protocol for an Randomized Controlled Trials (RCT) and two manuscripts belonged to the PubMed database and were all RCTs.
ConclusionNursing coaching improved QoL perceptions in cancer patients during their hospitalization. Patients were found to prefer in-person interventions to nurse-led ones, which improved QoL perceptions. However, further interventional studies need to be performed in order to better address coaching nursing interventions during the hospitalization of cancer patients.

Care Nursing in Immune Disorder Assessment among Adult Oncology Patients: A Scoping Review
BackgroundInternational guidelines recommend a pathway for preferable nursing handling in a specific cancer topic, like chemotherapy toxicity, low adhesion in toxicity reported with a consequential increase in adverse events (AEs) frequency, poorer QoL outcomes, and increased use of healthcare service until death. Unpredictability, postponed reports, and incapability to access healthcare services can compromise toxicity-related effects by including patients’ safety. In this scenario, a more attentive nursing intervention can improve patients’ outcomes and decrease costs for healthcare services, respectively. The present scoping review aims to describe and synthesize scientific care nursing evidence assessment in oncology patients.
MethodsPubMed, Embase, Nursing & Allied Health Database, and British Nursing were the databases examined. Keywords used and associated with Boolean operators were assessment, care, nursing, immune disorder, oncology, and patient. Research articles considered were published between 2013-2023. All systematic processes were performed according to the PRISMA procedure in order to reach all manuscripts considered in the present scoping review.
ResultsThe Embase database showed a total of 25 articles, PubMed displayed 77, the Nursing & Allied Health Database evidenced a total of 74, and the British Nursing database showed 252 records. Then, after a first revision in each database by considering the inclusion criteria, the abovementioned titles and abstracts were selected and, 336 records were removed, and 92 studies remained. Of these, 65 manuscripts were excluded after verifying abstracts. Finally, a total of 7 articles were carefully analysed and selected for this scoping review. Specifically, 2 articles belonged to the British Nursing Database, 3 articles belonged to Embase, 1 to the Nursing & Allied Health Database and one related to PubMed.
ConclusionOncology nursing should consider several aspects, such as therapy-related toxicity and its related morbidity and mortality, worsening levels of quality of life, and increasing duty by the healthcare organization or endorsements for the principal symptoms and signs which may anticipate few diseases and worst clinical conditions, too. Therefore, careful monitoring may allow prompt recognition and subsequent earlier management in the treatment efficacy.

The Impact of Post-traumatic Stress on Sexual Function of Survivors of Hematological Malignancies and Autologous Hematopoietic Stem Cell Transplantation
BackgroundAutolοgous Hematopoietic Stem Cell Transplantation (AHSCT) is a treatment οption fοr patients with hematological malignancies that improves prοgnοsis and survival. Thus, it is necessary tο deal with its long-term cοmplications, such as sexual dysfunction and trauma that significantly affect survivors’ quality οf life.
ObjectiveThe aim οf this study was to evaluate the Sexual Function (SF) and the Pοst-traumatic Stress Disorder (PTSD) οf survivοrs οf hematolοgical malignancy and AHSCT.
MethodsA multicenter, quantitative, crοss-sectional, descriptive, and cοrrelational study was cοnducted from December 2019 to March 2022. Thrοugh cοnvenience sampling, 127 adults and sexually active survivors οf hematolοgic malignancy whο underwent AHSCT frοm 6 mοnths tο 5 years were recruited frοm 5 hοspitals in Athens. The survivοrs cοmpleted questionnaires on demοgraphic and clinical data, a male or female SF assessment tοοl (Internatiοnal Index Erectile Functiοn (IIEF) and Female Sexual Functiοn Index (FSFI), respectively), and the Impact of Event Scale-Revised (IES-R).
ResultsPatients’ mean age was 45.6 (±12.8) years. The median time frοm transplant was 3 years and the majοrity οf the survivοrs had Hodgkin's lymphοma. Accοrding tο the FSFI, wοmen had a mοderate level of SF, with a better functional area the “pain” and wοrse the “οrgasm”. Accοrding tο the IIEF, men had a high level of SF, with a better functional area the “erection” and wοrse the “οverall satisfaction”. Survivοrs had lοw levels οf PTSD based οn the IES-R, with nο differences between men and women. The subscale οf “hyperarοusal” had the lοwest values and the subscale οf “avοidance” had the highest values fοr bοth men and wοmen survivοrs. There was a lοw tο mοderate negative statistically significant cοrrelation between the IES-R and bοth FSFI and IIEF, suggesting a high level οf PTSD to be correlated with wοrse sexual function.
ConclusionSurvivοrs οf AHSCT had impaired SF tο varying degrees and areas of functionality depending οn their gender, but lοw levels of PTSD and lοw correlations between these twο variables. This matter is impοrtant in survivοrship care and needs further investigatiοn.

M6a Methylation: A New Avenue for Control of the Development of Type 2 Diabetes Mellitus and its Complications

Therefore, there is an urgent requirement to investigate the pathogenesis of T2DM and its complications, which will be conducive to discovering effective drugs for prevention and treatment. N6-methyladenosine (m6A) methylation is the most abundant and prevalent epigenetic modification of mRNA in mammals. m6A methylation is a dynamically reversible epigenetic transcriptome modification process that is jointly regulated by methyltransferases, demethylases and methylated reading proteins, which control the fate of target mRNAs through influencing splicing, translation and decay. Recent studies have revealed that m6A methylation plays an important role in β cellular function, insulin sensitivity and glycolipid metabolism. In this review, we summarized the current roles of m6A methylation in T2DM and T2DM-related complications such as diabetes nephropathy (DN), diabetes cardiovascular disease (DCD) and diabetes retinopathy (DR). Additionally, we sought the potential mechanism of m6A in T2DM and related complications, which may provide a rationale and strategy for potential therapeutic targeting of T2DM and its complications.

Ketogenic Diet and Endocrine and Metabolic Diseases: A Bibliometric Study and Visualization Analysis

This study carried out bibliometric analysis to examine the research trends in this field over the past 20 years, aiming to provide insights for future studies.
MethodsWe searched the Web of Science Core Collection for all relevant papers. VOSviewer was used for network visualization, the bibliometrix package of R software (version 4.3.0) was utilized for data analysis, and CiteSpace was employed for mapping and trend analysis.
ResultsThis study encompassed 508 relevant articles spanning from 2003 to 2023, authored by 2827 researchers from 887 institutions across 57 countries/regions. The total number of publications increased from 3 in 2003 to 508 in 2023, showing a steady growth trend. The United States emerged as the predominant contributor in this field, followed by Italy and China. Notably, SAJOUX I consistently exhibited high activity in this field, according to the analysis, with an h-index of 13. The journal Nutrients has consistently made substantial contributions to this field, accounting for 19% of all publications. The keywords “obesity,” “ketogenic diet,” and “weight loss” appeared most frequently, with “obesity” occurring 163 times.
ConclusionThis study used a bibliometric method to analyze the impact of the ketogenic diet on the endocrine metabolic system. The research identifies recent frontiers and trending directions, providing valuable references for scholars in this field.

Bulky Isolated Adrenal Metastasis as First Presentation of Occult Hepatocellular Carcinoma (HCC) in a Patient with a Synchronous Squamous Carcinoma of the Tongue
Authors: Marco Lodin, Alberto Ragni, Valerio Renzelli, Maura Rossi, Elena Silvia Traverso and Marco GalloBackgroundThe diagnostic workup of an adrenal mass should always rule out the possibility of an adrenal metastasis, especially in a patient followed-up for a known primitive cancer. Sometimes, however, the incidental finding of a bulky lesion in a cancer patient can lead to the unexpected diagnosis of metastasis from a second occult cancer. Here, we report the case of a voluminous, isolated left adrenal metastasis from unknown and persistently occult hepatocellular carcinoma (HCC), incidentally found during the follow-up for squamous carcinoma of the tongue.
Case DescriptionA 72-year-old HBV/HCV-negative male patient with a history of alcohol abuse was referred to our hospital for gastric bleeding. Some weeks before, the patient was operated on for a locally advanced squamous cell carcinoma of the tongue, which required cervical lymph node neck dissection, temporary tracheostomy, jejunostomy, and plastic reconstruction. Subsequent diagnostic imaging revealed a left adrenal mass sized 9x15 cm with suspicious features. The hormonal workout was negative for pheochromocytoma and a hyperfunctioning adrenal lesion. The patient underwent laparotomic left adrenalectomy. The exploration of the liver was compatible with alcoholic cirrhosis and did not reveal any other palpable lesion. The adrenal mass histologically turned out to be a poorly differentiated G3 HCC. Subsequent radiological exams were unable to identify the primary liver lesion or any other neoplasms. Conversely, α-FP levels were initially high but reduced after treatment with sorafenib. After 2 years of follow-up, the patient is alive and well, albeit with evidence of locoregional inter-aortocaval lymphadenopathy. The primary HCC has never been identified, thus suggesting the hypothesis of a diffuse cirrhosis-like HCC.
ConclusionThe suspicion of an adrenal metastasis in a patient with primary cancer with a low potential for adrenal metastatic spreading must raise the diagnostic suspect for another synchronous occult cancer beyond that for primary adrenal cancer. HCC can rarely first manifest as a metastatic adrenal lesion.

Thyroid Hormone Resistance: A 17-Year Follow-up Case Report
BackgroundResistance to thyroid hormone is a rare syndrome characterized by peripheral resistance to thyroid hormones. It is caused by genetic dysfunction of thyroid receptor genes, with Thyroid hormone Receptor-beta (TRβ) being the most prevalent. Affected patients show high thyroid hormone levels and non-suppressed Thyroid-stimulating Hormone (TSH). Syndrome manifestations vary from hyperthyroidism to hypothyroidism depending on the specific mutation.
Case PresentationWe, herein, describe the case of a 24-year-old female with a diagnosis of resistance to thyroid hormone from the age of 7. The main symptoms the patients complained about were headaches, palpitations, hyperidrosis, and frequent evacuations with severe underweight. The patient’s blood test showed high FT3 and FT4 levels with a non-suppressed TSH. We performed a disease complications screening that revealed mild osteoporosis and normal cardiac activity (the patient was already treated with bisoprolol).
ConclusionThis case illustrates symptoms and complications of resistance to thyroid hormone syndrome, a rare and misdiagnosed condition. In this case report, we describe and explain long-term disease symptoms and their management. The long-term history of our patient’s disease adds a more comprehensive evaluation of the syndrome and its consequences, contributing to new insights into the resistance to thyroid hormone syndrome and shedding light on personalized management of its manifestations.
